Introducing the Care Team Co-Management App: Streamlining Collaboration for Enhanced Patient Care
The new Care Team Co-Management App is designed to make it possible for providers to work more closely as a team with other providers. This App makes it easier to share useful patient health information in order to improve patient outcomes and bring a higher level of efficiency to their practices.
The Care Team Co-Management App has three purposes:
- To provide structured fields, where EHRs currently do not. Structured fields for specialty areas in eyecare such as dry eye, diabetes and cataract surgery.
- To standardize, in a single platform, the way providers record specialty.
- To reduce or eliminate the barriers to coordinating care in specialty areas of eyecare.
