2. During the demo, we’ll discuss your specific practice needs, and provide a tailored subscription price.
3. We’ll then provide a Subscription Registration Form for you to complete and return, then we will send your login credentials to HealthCare Registries software. 
4. We will then send you a Data Extraction Authorization Form to complete and return to us. 
5. Once received, we will forward this form to your EHR vendor, who will provide us with credentials to access your EHR. 
6. Our support team will begin extracting data from your EHR.

No. A number of years ago, the AOA launched a registry called MORE. Their registry is now defunct. The American Society of Ophthalmology has a registry called IRIS which is primarily focused on MIPS measures.
