Instructions to View Reports
Steps:
- Go to HealthcareRegistries.com
- Click “Log In” located top right tool bar
- Enter Username: available by request – ContactUs@Healthcareregistries.com
- Enter Password:
- Click Log in button
- Click “Reports” dropdown menu. Only those links highlighted in yellow are active since we do not have data on the other analytics from your office yet,

- You are able to see registry level analytics to understand the concepts

- You are able to view the data from a de-identified office to see how their analytics work

Important– it takes a few seconds for a report to load because the data is refreshed everyday
My Summary
Graph and data display 1 Year Patient Count in the following categories:
Diabetes, Myopia, DryEye, DryEyeCataract, Cataract and Glaucoma

- AMD- Summary
- Compares % wet vs dry and age. Also compares Registry data to your Organization
- AMD Outcome – VA by Medication
- Graph displays VA progress over last 3 month for only AMD Dry patients
- AMD- Summary
- Quality Assessment
- Anisocoria -graph and data displays Total # of Patients Seen and # of times and % Anisocoria has been diagnosed
- Hypertension – graph and data display Total #of Patients Seen and # of time Hypertension (HT) and Hypertensive Retinopathy (HTR) has been diagnosed
- Hypertensive Retinopathy – same as Hypertension

- Patient by Dx – displays Dx Code, # of Patient Encounters and Dx Description. Note Dx code HO4.123, Dry Eye Syndrome of Bilateral Lacrimal Gland, 820 encounters.
- Patient Attrition – displays the # of patients, in each Dx category, that have not returned to your office in the last 12+ months
- Patients – displays the # of patients seen in last 12 months. By clicking on an underlined number, for example 115944525, will launch another display to provide you with more diagnosis and visual acuity of this patient.
